Why we're asking

A team picks an AI tool, the demo looks great, and then it meets the company's real data.

Customer records spelled four different ways. A CRM and a billing tool that disagree on what counts as an "active" account. A spreadsheet only one person knows how to update. A few months later the pilot is quietly parked and the tool takes the blame.

Operators describe some version of this over and over. We want to find out how often it happens, where it hurts most, and whether leadership sees it coming.

What the survey asks about

Every question is multiple choice or a quick scale, except the last one. None of them ask for your name or your company's name.
